Notice of Privacy Practices

This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our duties

Klutch Therapy is required by law to maintain the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to tell you if a breach affects your information.

How we may use and disclose your information

Treatment. To provide, coordinate and manage your care, including with other providers involved in your care.
Payment. To bill and collect for services, including with your health plan.
Health care operations. To run the practice, such as quality review, training and scheduling.
As required or permitted by law. For example, to report suspected abuse or neglect, to respond to a court order, or to prevent a serious and imminent threat to health or safety.
With your written authorization. Most other uses and disclosures, and any use of psychotherapy notes beyond what the law allows, require your written permission, which you may withdraw at any time.

Your rights

To see and get a copy of your records.
To ask us to correct information you believe is wrong or incomplete.
To ask for confidential communications, such as a different phone number or address.
To ask us to limit certain uses and disclosures. We will tell you if we cannot agree.
To receive a list of certain disclosures we have made.
To receive a paper copy of this notice at any time.

Questions and complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. Changes to this notice

We may change this notice and the change may apply to information we already hold. The current notice will always be posted on this page and available on request.
